Teknik Radiografi Intra Venous Pyelography (IVP)

1. Definisi
Ilmu yang mempelajari prosedur /tata cara pemeriksaan ginjal, ureter, dan blass (vesica urinary) menggunakan sinar-x dengan melakukan injeksi media kontras melalui vena.
  • Pada saat media kontras diinjeksikan melalui pembuluh vena pada tangan pasien, media kontras akan mengikuti peredaran darah dan dikumpulkan dalam ginjal dan tractus urinary, sehingga ginjal dan tractus urinary menjadi berwarna putih.
  • Dengan IVP, radiologist dapat melihat dan mengetahui anatomy serta fungsi ginjal, ureter dan blass.
2.  Tujuan Pemeriksaan IVP
  • Pemeriksaan IVP membantu dokter mengetahui adanya kelainan pada sistem urinary, dengan melihat kerja ginjal dan sistem urinary pasien.
  • Pemeriksaan ini dipergunakan untuk mengetahui gejala seperti kencing darah (hematuri) dan sakit pada daerah punggung.
  • Dengan IVP dokter dapat mengetahui adanya kelainan pada sistem tractus urinary dari :
    • batu ginjal 
    • pembesaran prostat
    • Tumor pada ginjal, ureter dan blass.
3. Indikasi Pemeriksaan IVP
  1. Renal agenesis
  2. Polyuria 
  3. BPH (benign prostatic hyperplasia)
  4. Congenital anomali : 
    • duplication of ureter n renal pelvis
    • ectopia kidney
    • horseshoe kidney 
    • malroration
  5. Hydroneprosis 
  6. Pyelonepritis 
  7. Renal hypertention
4. Kontra Indikasi
  • Alergi terhadap media kontras
  • Pasien yang mempunyai kelainan atau penyakit jantung
  • Pasien dengan riwayat atau dalam serangan jantung 
  • Multi myeloma
  • Neonatus 
  • Diabetes mellitus tidak terkontrol/parah
  • Pasien yang sedang dalam keadaan kolik
  • Hasil ureum dan creatinin tidak normal
5. Persiapan Pemeriksaan
  1. Persiapan Pasien 
    1. Pasien makan bubur kecap saja sejak 2 hari (48 jam) sebelum pemeriksaan BNO-IVP dilakukan.
    2. Pasien tidak boleh minum susu, makan telur serta sayur-sayuran yang berserat.
    3. Jam 20.00 pasien minum garam inggris (magnesium sulfat), dicampur 1 gelas air matang untuk urus-urus, disertai minum air putih 1-2 gelas, terus puasa.
    4. Selama puasa pasien dianjurkan untuk tidak merokok dan banyak bicara guna meminimalisir udara dalam usus. 
    5. Jam 08.00 pasien datang ke unit radiologi untuk dilakukan pemeriksaan, dan sebelum pemeriksaan dimulai pasien diminta buang air kecil untuk mengosongkan blass.
    6. Yang terakhir adalah penjelasan kepada keluarga pasien mengenai prosedur yang akan dilakukan dan penandatanganan informed consent.
  2. Persiapan Media Kontras
    • Media kontras yang digunakan adalah yang berbahan iodium, dimana jumlahnya disesuaikan dengan berat badan pasien, yakni 1-2 cc/kg berat badan.
  3. Persiapan Alat dan Bahan
    1. Peralatan Steril
      • Wings needle No. 21 G (1 buah)
      • Spuit 20 cc (2 buah)
      • Kapas alcohol atau wipes 
      • Tourniquet 
    2. Peralatan Un-Steril
      • Plester
      • Marker R/L dan marker waktu
      • Media kontras Iopamiro (± 40 – 50 cc)
      • Obat-obatan emergency (antisipasi alergi media kontras)
      • Baju pasien
6. Prosedur Pemeriksaan BNO-IVP
  1. Lakukan pemeriksaan BNO posisi AP, untuk melihat persiapan pasien
  2. Jika persiapan pasien baik/bersih, suntikkan media kontras melalui intravena 1 cc saja, diamkan sesaat untuk melihat reaksi alergis.
  3. Jika tidak ada reaksi alergis penyuntikan dapat dilanjutkan dengan memasang alat compressive ureter terlebih dahulu di sekitar SIAS kanan dan kiri.
  4. Setelah itu lakukan foto nephogram dengan posisi AP supine 1 menit setelah injeksi media kontras untuk melihat masuknya media kontras ke collecting sistem, terutama pada pasien hypertensi dan anak-anak.
  5. Lakukan foto 5 menit post injeksi dengan posisi AP supine menggunakan ukuran film 24 x 30 untuk melihat pelviocaliseal dan ureter proximal terisi media kontras.
  6. Foto 15 menit post injeksi dengan posisi AP supine menggunakan film 24 x 30 mencakup gambaran pelviocalyseal, ureter dan bladder mulai terisi media kontras
  7. Foto 30 menit post injeksi dengan posisi AP supine melihat gambaran bladder terisi penuh media kontras. Film yang digunakan ukuran 30 x 40.
  8. Setelah semua foto sudah dikonsulkan kepada dokter spesialis radiologi, biasanya dibuat foto blast oblique untuk melihat prostate (umumnya pada pasien yang lanjut usia). 
  9. Yang terakhir lakukan foto post void dengan posisi AP supine atau erect untuk melihat kelainan kecil yang mungkin terjadi di daerah bladder. Dengan posisi erect dapat menunjukan adanya ren mobile (pergerakan ginjal yang tidak normal) pada kasus pos hematuri.
7. Kriteria Gambar 
  1. Foto 5 menit post injeksi
    • Tampak kontras mengisi ginjal kanan dan kiri.
  2. Foto 15 menit post injeksi 
    • Tampak kontras mengisi ginjal, ureter.
  3. Foto 30 menit post injeksi (full blass)
    • Tampak blass terisi penuh oleh kontras 
  4. Foto Post Mixi 
    • Tampak blass yang telah kosong.
8. Perawatan Lanjutan
Tidak ada perawatan khusus yang diberikan kepada pasien setelah menjalani pemeriksaan BNO-IVP ini.
Catatan :
KELEBIHAN DAN KEKURANGAN IVP
  • Kelebihan 
    1. Bersifat invasif.
    2. IVP memberikan gambaran dan informasi yang jelas, sehingga dokter dapat mendiagnosa dan memberikan pengobatan yang tepat mulai dari adanya batu ginjal hingga kanker tanpa harus melakukan pembedahan
    3. Diagnosa kelainan tentang kerusakan dan adanya batu pada ginjal dapat dilakukan.
    4. Radiasi relative rendah 5. relative aman
  • Kekurangan 
    1. Selalu ada kemungkinan terjadinya kanker akibat paparan radiasi yang diperoleh.
    2. Dosis efektif pemeriksaan IVP adalah 3 mSv, sama dengan rata-rata radiasi yang diterima dari alam dalam satu tahun.
    3. Penggunaan media kontras dalam IVP dapat menyebabkan efek alergi pada pasien, yang menyebabkan pasien harus mendapatkan pengobatan lanjut. 
    4. Tidak dapat dilakukan pada wanita hamil.

Basic Anatomy

Indications for imaging
Suspected urinary tract pathology.
Repeated infections ? focus, damage, (when linked with other symptoms.)
Heamaturia.
Investigation of hypertension not controlled by medication in young adults.
Renal colic.
Trauma.
Contra Indications:
General contra indications to water soluble contrast agents.
Hepato renal syndrome,
Thyrotoxicosis,
Pregnancy, (Allow 28 days from childbirth)
Blood urea raised above 12 mmol./L. urography unlikely to be successful.

Patients Preparation:
Basic abdominal preparation, aperients taken for 24 hours previous, to clear faecal residue.
Nil by mouth for 4-6 hours before the examination.
Patient to remain ambulant as long as possible to reduce air swallowing.
Adaptations to patient preparation will be required for certain groups of patients e.g. children, diabetics and patients with other predisposing medical conditions, in line with current department practice.
Basic Patient Position
The patient lies supine on the table midline aligned to the midline of the table.

Central Ray
Control Film of abdomen

The vertical central ray is centered in the midline at the level of L3 at the lower costal margin,
It may be necessary to alter the centering position to ensure the upper margin of the renal outlines are visible and the symphysis pubis.
Exposure is made on suspended expiration.

Renal area films
The vertical central ray is centered in the midline at the level of the midpoint of the upper and lower borders of the renal outlines.
Exposure is made on suspended expiration.
Bladder Films
The central ray is angles 15 degrees caudally and centered in the midline to a point at a level midway between the symphysis pubis and the ASIS.
Exposure is made on suspended expiration.

Radiation protection
Direct lead rubber gonad protection using a "half apron" or Kings Lyn shields when possible.
Film Sequence
Preliminary film (Control), (35 x 43cm) supine full A.P. abdomen to include lower border of symphysis pubis and diaphragm, to check, abdominal preparation, exposure values and for any calcifications overlying the renal tract areas.
Supplementary films to determine position of any opacities.
Inspiration film of renal areas for suspected calculi.
35° posterior oblique of the renal areas.
Tomogram of the renal areas at 8-11cm.

Contrast Media Injection
The median cubital vein is punctured with a 19 gauge needle and the warmed (40*C) contrast agent is injected rapidly. Films are then taken at intervals to demonstrate the whole of the renal tract.

End of Injection, (24 x 30cm) A.P. of the renal areas to show the nephrogram, i.e. the renal parenchyma opacified by the contrast medium in the renal tubules.

5 Minute film, (24 x 30cm) A.P. of the renal areas to determine if excretion is symmetrical or if uptake is poor and a further dose of contrast agent is required.
.

Compression may be applied in some centers at this point to distend the pelvicalyceal systems to demonstrate any filling defects and a film taken at 10 minutes of the renal areas. Compression should not be used in cases of suspected renal colic, renal trauma or after recent abdominal surgery.

15 Minute film (35 x 43cm) (On release if compression has been applied) to demonstrate the pelvicalyceal systems and the ureters.


25 Minute film (24 x 30cm) 15° caudal angulation centered 5 cm above the upper border of the symphysis pubis to demonstrate the distended bladder.


Post Micturition film (24 x 30cm) 15° caudal angulation centered 5 cm above the upper border of the symphysis pubis to demonstrate the bladder emptying success, and the return of the previously distended lower ends of ureters to normal.


Patient Aftercare
General psychological reassurance.
Needle wound site dressed and checked for extravasation.
Check patient understands how to receive the results.
Ensure patient understands any preparation instructions are finished
Escort to changing rooms and bid good-bye.

Evaluation of the Image
ID and markers must be present and correct in the appropriate area of the film
Evidence of collimation on four sides equally around the centering point
Optimal exposure should just penetrate all the contrast media filled structures and contrast should be optimised  to visualise fully the renal and soft tissue structures.

Related Projections
Inspiratory, expiratory and oblique projections may be required to demonstrate the relationship of opacities and filling defects to the renal tract.
Tomography, may be required to accurately demonstrate the renal outlines and overcome shadowing from the gastro intestinal tract.
Prone films may be required to investigate pelvi ureteric and ureteric obstruction as the heavy contrast laden urine will more readily gravitate to the site of the obstruction.
Rapid sequence films may be taken in cases of suspected renal hypertension to evaluate differential rates of contrast excretion.
Delayed films may be taken for up to 24 hours in order to demonstrate the actual site of ureteric obstruction.
Additional modalities
Radio nuclide imaging for renal function evaluation.
Radio nuclide imaging for renal transplant rejection assessment.
Ultrasound for bladder investigation as a first line of imaging.
Ultrasound for renal cyst imaging and drainage if required.
C.T. for investigation of trauma and renal masses.
Renal Angiography.
Retrograde pyelography,
Urethrography.
Magnetic resonance imaging.
